Click on the green text that states '**Consent form'**. ![](https://tattoogenda.com/wp-content/uploads/2023/08/consent-form-06.png) ![](https://tattoogenda.com/wp-content/uploads/2023/06/Wide-splash-small-24-1024x78.png) When clicking on the green Consent form link we will get a pop up with a detailed version of the consent form.
